Coordination of Treatment
If you are a psychiatrist, primary care physician, school social worker, or other professional looking to coordinate treatment with one of our therapists, please have your client (if over the age of 12) and their parent or guardian complete an Authorization to Release Information form and send a copy to [email protected].

Conference Room Rental
Our Bucktown conference room, by the hour or the day, for workshops, consults, or client groups. Seats up to 12. Provider Hub members save 15%.
